Please help me identify this book~

I read a book years ago in grade school, but cannot for the life of me remember the title or the author. Without any means to find them, I decided to start here, where I have seen other posters aided in finding information on books they have once read.

Here I will list the bits and pieces of what I can remember:

x) It very possibly had sequels, but I read only the first of the books.
x) The protagonist was male, was young at the beginning of the book, and had a sort of nanny.
x) There is a talent that the boy must learn, and it might be something that runs in the family.
x) Basically, the talent consists of going to sleep, but while asleep waking up in another world. In the other world pain is very real, and death there would mean death in the physical as well. It is very possible than pain and injuries passed through the worlds, but I am not sure.
x) At first the boy is not able to perfrom the skill to leave the world. He goes to a doctor of sorts to figure out what the problem was. Among other things, the doctor said that metal could be what was holding him back, and had him remove his watch and things.
x) I believe that it turned out to be his braces, or some kind of metal in his mouth that hampered him.
x) When the boy finally was able to go into the other world, he was given jobs. These jobs consisted of delivering packages, but he was not allowed to find out what the contents were. If I recall correctly, he later found out that the contents were bad, or illegal.
x) Later in the book, I seem to remember him entering some sort of room in the other world, and being trapped. I remember that he interacted with another person while trapped, but I'm not quite sure what became of this.
x) At the end of the book, I remember some sort of great battle, prehaps involving the main character having the defend a castle of sorts.
x) I think I came to the book by being recommended to it by a paper in my library. I don't think it can be very obscure.

Those are the things I can remember from the book. With luck, it really is one book that I am not remembering, and not just portions from many books I have combined into one great story in my head!
