Should I include a separate title page on short story manuscripts?

No, you can just put the title at the top of the first page.

If you are submitting to a particular market you may want to check their guidelines. If for no other reason, some of these people have spent a lot of time writing their guidelines, and it's only polite to read them once through. Sometimes you can also pick up on their general attitude toward submitters.
Of course the intent is a clear, clean and easy to read manuscript; and sometimes, easy to transfer to their printing program.

Rather than start a new thread...
What about footnotes? How do you do footnotes in a manuscript? Just like they would appear normally? As endnotes? Endnotes at the end of the chapter, or at the end of the text?

Footnote the text and then have a separate page or pages inserted with the footnote text in the same size print as the regular text and probably double-spaced as well. If you don't want to mess up your page count, you could call the footnote pages 1A and such.
