Well, both Donaldson and Banks write some disturbing books, but they aren't really 'sick'.
The only books I can think of that fit are Graham Masterton's Knight Warriors trilogy (though I've only read the first two). Some disturbing stuff there.

Never read any "sick" books, or at least none that I would categorize as such, but I've read a few that made me cringe. A simple plan, at the end, was quite descriptive on a particular death scene, but even then, it didn't go too far, it was just rather gross. I remember reading in a book (can't remember the title at the moment...I know it was historical fiction though) that went on in a lot of detail about how to flush and drain an infected wound, and thn described exactly what Maggot cleansing feels like (where they use maggots in wounds to eat away the dead and infected skin, because they will not eat live flesh). But again, although it was kind of stomach turning at times, I wouldn't call it sick.
I'm just going to shutup now. I don't think I can think of anything that I would just consider sick. I'm not a very easily offended person.

Easily Imajica, by Clive Barker. We're talking sexual depravity, here. I've ranted about this book before.

Boys of Life. There was a time in high school where I read a lot of books similar to this, but Boys of Life... there were some things about that book that were truly, truly sick. It was an amazing, stick in your head kind of book, but it was graphic. Perhaps it's because I'm a heterosexual female, but some of the really descriptive scenes of gay male ... interaction ... kind of squicked me. And I still have very vivid images of one of the death scenes. Truly disturbing. Sea of Tranquility was also disturbing, but not to the same extent.
Also, everything by Poppy Z Brite. She's just terrible, but it's more that she's a terrible writer. She writes some messed up stuff, but you have to laugh at it. Everything is "like blood left too long in a stoppered bottle." (And yet I've read almost everything she's written. It was a phase.)

American Psycho. There's a lot of rather sick violence and torture in there. You don't even want to know what he does with the starving rat.
