Help with remembering book/author...

I'm trying to recall the author and title of a book (and possibly a series) that I read a very long time ago. I only have a couple of weird memories of what I read, none of which have helped me find the title so far, but perhaps someone here can help.

One scene I remember is the protagonist had found himself forced to fight within a Norse-like army who then storm a beach and attack an Aztec-like army. The leader of the Norse-like army wields a magical hammer. The protagonist gets a small injury during battle and considers using it as an excuse to fall back, but notices other warriors limping ahead with spears through their legs and realises that the Norse-like people do not accept retreat and continues onward himself.

Another aspect I recall is that when the protagonist went to sleep in the fantasy world, he would wake in the 'real' world (our world). During the time he was awake in the fantasy world, the real world continues and his two personas (the one in the fantasy world and one in the real world) would 'merge' whenever he went to sleep in the fantasy world. He could also bring some stuff back with him from the real world, like food.

If anyone could please help me remember the author, title or series, I'd be grateful. Thanks!

It sounds like the Everworld series by K. A. Applegate to me. It had the protagonists escaping from Norse gods in the first book and fighting Aztecs in the second. It was set in another world, but the protagonists were from our world and I seem to remember them coming back to our world when they sleep. One of the protagonists' name was April.
