Did #3 of this mini-series ever come out? It was written by Warren Ellis, and published under Image.

Any ideas on what happened to it?

No, it hasn't been published yet. Ellis had some health problems and so didn't get the script out on time. Now, Chris Weston is busy pencilling Grant Morrison's The Filth for Vertigo. He's working on MoS in his "spare" time. It'll hopefully be out sometime this year.
