If it was a wish spell, a la Aladdin, Dragonball, etc... someone would totally summon something fuzzy and cute, like a chinchilla. Because that's more zany than a rabbit or teddy bear (hehe, Muse video for Uprising with the demon teddy bear).
Something simple, like a child that spreads a disease, makes people barren.
Simple yer human, so much better than a big thing.

But, as Igor said, sometimes the simplest thing can seem so horrendous - like maggots and/or presidential elections.

If I wanted to control the world through fear, I would summon infinite amounts of Money.

I'm with Igor on this - a single big monster can only be in one place at a time (presumably), and it's a fairly safe bet that anything alive can eventually be killed somehow.
Plague is so much nastier. Better than a plague that kills people, how about a plague that kills the plants? Soon you have no crops, and no food animals, and mass starvation. The decent into anarchy and cannibalism ensues quite fast from that point, I would imagine.
Personally I find that sort of thing an awful lot scarier than a monster!
What? No blunt hammer like a tarasque, Cthulu or some sort of monster that burps black holes? :-P
The problem there, is that it's the nuclear option - once that sucker appears, there's nowhere to go (or rather, that's the point when our hero gets out the magic sword/discovers he had the power within him all along/tricks the baddie into destroying himself), so that kind of monster is automatically the end-point of a conventional story. Is that what you're after, SilentDan?

Yeah, it's a nuke kind of monster, ie: the endgame entity. That might make an alright title... or maybe not.
